**Why Avoid under the strict bar:** Novel systemic enzyme inhibitor with zero human safety data; unknown long-term metabolic and methylation effects.

## What users report

Some swear it offsets GLP-1 fatigue and 'shreds abs'. Just as many ran it 11 weeks, or 50-100 mg oral, without being able to tell. Nearly everyone confounds it with retatrutide, tesamorelin and diet. Reconstituted stability is poor, and a few report reactions. Roughly half feel nothing.

**Effects:** energy (anecdotal); fat loss alongside GLP-1s (confounded)

**Downsides:** no clear effect for many; unstable once reconstituted; injection reactions; expensive at high doses

**Risks:** No human safety data at all; chronic systemic enzyme inhibition alters methylation (SAM) and NAD+ salvage pathways with unknown long-term effects; Not lawful for 503B compounding (FDA warning letter Jan 2026)

**Typical use:** 50-150 mg/day oral, or 1-20 mg injected (no consensus).

**Getting it:** Research vials (5-50 mg), oral capsules and bulk powder sold as a 'peptide' though it is a small molecule; FDA has told a 503B outsourcer it is not eligible for compounding.

**Stacked with:** Retatrutide or other GLP-1s (The main use case: taken to offset GLP-1 fatigue and speed fat loss, which leaves its own effect unmeasurable.); NMN or NAD+ (Rationale is that blocking NNMT spares nicotinamide for NAD+; one oral user only felt it after adding 1 g/day NMN.); MOTS-c and SS-31 (Cycled with or after MOTS-c in 'mitochondrial' stacks; several users cannot tell the compounds apart.)

## Risk profile

- **Seen harm 0/3:** No human study exists; the two bad first-hand reports (a blood pressure of 77/56 after injecting it with three other compounds, and two days of diuresis from a product of doubtful identity) are confounded and resolved.
- **Mechanism 1/3:** A small-molecule NNMT inhibitor that shifts methyl-group (SAM) and nicotinamide metabolism; NNMT inhibition is usually framed as anti-tumour, and the long-term effect on methylation is unknown rather than shown harmful.
- **Data gap 3/3:** No human data of any kind; efficacy and safety work is rodent-only, from the developer's group.
- **Silent harm 2/3:** Chronic changes in methylation or NAD metabolism would build silently and show, if at all, only on specialised tests.
- **Product 3/3:** Most reports describe injecting research vials, with capsules second; FDA told a 503B outsourcer it cannot be compounded.
- **Never if:** Pregnancy or breastfeeding (no human reproductive data)

## Top reports

- Glowing, metabolic, 2 mg/day rising to 5 mg/day: “Paired with Reta, it definitely counteracts the sluggishness one feels in the first two months of taking any GLP agonist. Once this stuff builds up in your system, it really does give you a good clean boost of daily energy without affecting your sleep.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1n6osh6/review_5amino1mq_injections/)
- Glowing, energy, 1 mg: “Been taking mots-c on its own and haven't felt much. Added 1mg 5amino yesterday and had my hardest workout and had to drag myself out of the gym. Then I got home and immediately went for a run.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1n6osh6/review_5amino1mq_injections/nc3t5m8/)
- Glowing, metabolic: “I’ve done five cycles of 5 amino started with the pills first little to no effect, then started high dose injections and got great results good energy great fat loss around my mid section and all over and never had any side effects” — [r/PeptidePathways](https://www.reddit.com/r/PeptidePathways/comments/1utybpt/are_there_any_potential_risks_associated_with_5/ox9q0zx/)
- Positive, energy, 4 mg/day: “But, I've been using 4mg daily. Combined with my Lipo-c, L-carnitine and SS-31, my energy levels during my workouts and chopping firewood has been fantastic!” — [r/Biohacking](https://www.reddit.com/r/Biohacking/comments/1xg2sgc/first_impression_5amino/)
- Positive, energy, 5 mg/day: “5mg for 12 weeks here. Off it now but wish I stayed on. I had no side effects and it was simply a subtle boost in energy all day, early pin and felt normal tired at 10pm as expected. I praise it.” — [r/Biohacking](https://www.reddit.com/r/Biohacking/comments/1w7l9vs/5_amino_1mq_your_thoughts/p82yg1v/)

## Studies

- [Neelakantan et al. 2018: selective and membrane-permeable small-molecule NNMT inhibitors reverse high-fat-diet-induced obesity in mice](https://pubmed.ncbi.nlm.nih.gov/29155147/) (animal, 2018): A methylquinolinium NNMT inhibitor (5-amino-1MQ) raised NAD+ and SAM in fat cells and, injected into diet-induced obese mice, cut body weight and white-fat mass, shrank fat cells and lowered cholesterol without changing food intake.
- [Neelakantan et al. 2019: NNMT inhibitor activates senescent muscle stem cells and improves regenerative capacity of aged skeletal muscle](https://pubmed.ncbi.nlm.nih.gov/30753815/) (animal, 2019): In 24-month-old mice with injured muscle, the inhibitor gave nearly twice the regenerated fibre cross-section and about 70% more peak torque than controls.
- [Sampson et al. 2021: combined NNMT inhibition and reduced-calorie diet normalizes body composition in obese mice](https://pubmed.ncbi.nlm.nih.gov/33707534/) (animal, 2021): Adding 5-amino-1MQ to a calorie-restricted diet gave more fat loss and better glucose tolerance than diet alone, bringing obese mice to lean-control body composition.
- [Kraus et al. 2014: nicotinamide N-methyltransferase knockdown protects against diet-induced obesity](https://pubmed.ncbi.nlm.nih.gov/24717514/) (animal, 2014): Knocking down NNMT in fat and liver raised energy expenditure, SAM and NAD+ levels and protected mice from obesity; the target-validation paper behind the compound.
- [Babula et al. 2024: NNMT inhibition mitigates obesity-related metabolic dysfunction](https://pubmed.ncbi.nlm.nih.gov/39161060/) (animal, 2024): 28 days of daily 5A1MQ in obese mice limited fat gain, improved insulin sensitivity and fatty liver, with pharmacokinetics for oral and subq dosing. By the developer (Ridgeline Therapeutics); no human trial of 5-amino-1MQ has been published.
