- Risk axes (0 none to 3 severe): Seen harm 1, Mechanism 2, Data gap 3, Silent harm 3, Product 3
- Positive: 46% of 53 graded reports glowing or positive, shrunk toward the 49% all-report average by 8 reports' weight
- Rank: #41 of 61 research peptides

- Evidence: Small human studies — Plus tiny human pilots and case series
- Access: Grey market
- Onset: Acute injuries: less pain within days to 2 weeks; chronic tendon or joint problems usually 3-6 weeks. Oral use for gut symptoms is often felt within the first week.
- Lasts: Half-life under 30 minutes, so it is dosed once or twice daily. Healed injuries stay healed; arthritis-type pain often returns within weeks of stopping.

- Dose users report: 500–880 mcg/day (median 500 mcg, n=10)
- Updated: 2026-10-30

**Why Avoid under the strict bar:** Pro-angiogenic mechanism with a plausible path to tumour growth and retinal neovascularisation, plus essentially no human safety data. HHS enforcement discretion (29 Oct 2026) is not a safety finding.

## What users report

Users report strong, often dramatic benefits: tendon/rotator-cuff and back pain relief, surgeons 'shocked' at recovery speed, gut/reflux fixes. Acute injuries respond better than chronic ones. Roughly 20-25% report nothing. A notable minority report anxiety, insomnia, anhedonia, headaches and flu-like reactions (often blamed on endotoxin-contaminated vials). There is also a growing cluster of eye complaints (blurry vision, eye pain), including one choroidal neovascularisation case that needed anti-VEGF shots.

**Effects:** faster tendon/ligament/muscle recovery; post-surgical healing; joint and back pain relief; gut lining / reflux / IBD relief

**Downsides:** anxiety, insomnia, 'speedy' feeling; anhedonia / low mood in some; headaches, nausea, dizziness; injection-site irritation

**Risks:** Pro-angiogenic (VEGFR2/eNOS) in animal models: theoretical promotion of existing tumours; Recurring eye/retina reports incl. a choroidal neovascularisation case (anecdotal, plausible via VEGF); No human safety data beyond tiny pilots (n=2 IV study); FDA CDER recommended against listing; Generic: grey-market endotoxin contamination is common

**Typical use:** 250-500 mcg/day subQ for 4-8 weeks (often 2x/day near the injury); 500 mcg oral for gut.

**Getting it:** Research-peptide vials and capsules; since HHS enforcement discretion (29 Oct 2026) US 503A pharmacies and telehealth clinics compound it. Banned in sport by WADA.

**Stacked with:** TB-500 (The 'Wolverine' stack, sold as a pre-mixed blend; most injury reports on Reddit are for this pair rather than BPC alone.); GHK-Cu and KPV ('GLOW' (BPC, TB-500, GHK-Cu) and 'KLOW' (plus KPV) blends; users also add BPC to injectable GHK-Cu to cut the sting.); KPV (Paired for gut and autoimmune inflammation, and used as the non-angiogenic substitute by people who quit BPC.)

## Risk profile

- **Seen harm 1/3:** FDA's July 2026 review found no published case reports and no serious events in the small human studies; what exists is isolated and confounded: FAERS reports involving blends, TGA hypersensitivity cases it could not split from TB-500, and a first-hand choroidal neovascularisation that needed anti-VEGF injections.
- **Mechanism 2/3:** Pro-angiogenic in vivo: it raised vessel density and blood-flow recovery in rat ischaemia by activating and up-regulating VEGFR2. No study has tested tumour promotion, and some report the opposite, so the concern is credible but undemonstrated; rat and dog toxicology also showed clotting-time and liver changes.
- **Data gap 3/3:** No human has received it orally, subcutaneously or nasally in a published study; the only human data are about 80 people given it rectally, into a joint or bladder, or IV (two people, both already exposed), in short studies from overlapping authors. No carcinogenicity study exists.
- **Silent harm 3/3:** The feared harms, promotion of an existing tumour or retinal neovascularisation, would be silent until advanced, which testimonials cannot reveal.
- **Product 3/3:** Most users inject grey-market vials (oral capsules are second); compounding became legal in the US only with the October 2026 HHS enforcement discretion.
- **Never if:** Active cancer or a history of cancer; Diabetic retinopathy, macular degeneration or other neovascular eye disease; Pregnancy or breastfeeding (no human reproductive data)

- [Lee et al. 2024: effect of BPC-157 on symptoms in patients with interstitial cystitis, a pilot study](https://pubmed.ncbi.nlm.nih.gov/39325560/) (open-label, n=12, 2024): A single 10 mg injection into the bladder wall during cystoscopy; 10 of 12 women reported complete symptom resolution and no adverse events were recorded.
- [Lee & Burgess 2025: safety of intravenous infusion of BPC157 in humans, a pilot study](https://pubmed.ncbi.nlm.nih.gov/40131143/) (open-label, n=2, 2025): 10 mg then 20 mg IV caused no change in heart, liver, kidney, thyroid or glucose markers in two adults who had used it before; far too small to establish safety.
- [Staresinic et al. 2003: BPC 157 accelerates healing of transected rat Achilles tendon](https://pubmed.ncbi.nlm.nih.gov/14554208/) (animal, 2003): Daily BPC-157 improved biomechanical, functional and histological healing of cut Achilles tendons in rats and stimulated tendon-cell growth in vitro.
- [Hsieh et al. 2017: therapeutic potential of pro-angiogenic BPC157 is associated with VEGFR2 activation and up-regulation](https://pubmed.ncbi.nlm.nih.gov/27847966/) (animal, 2017): BPC-157 raised VEGFR2 expression and signalling, sped blood-flow recovery and grew new vessels in a rat limb-ischaemia model; the same mechanism underlies the tumour and retinal concerns.
