## What users report

Typical report is a 'warm fuzzy' social openness lasting 30 min to a few hours, described as underwhelming by many; autistic adults on daily long-term use describe effects as very subtle but real. Perhaps 40% notice little. Some find it intensifies emotions or upsets the stomach; libido/orgasm boosts are a recurring positive.

**Effects:** social warmth and openness; less social/background anxiety; better orgasms/libido; mild reduction in rumination

**Downsides:** effects short and subtle; can intensify negative emotions in some; stomach upset (isolated); nasal soreness with daily use; diminishing returns on same-day redosing

**Caveat:** Systematic review of 38 RCTs (n=1529) found no reliable side effects at 18-40 IU short-term. Real but unlikely at these doses: water retention/low sodium at very high doses, and it is uterotonic, so not for pregnancy. Available on prescription, so purity can be pharmacy-grade.

**Typical use:** 12-40 IU nasal as needed (research standard 24 IU); not daily for most.

**Getting it:** Off-label compounded nasal spray on prescription in the US; grey-market vials and sprays elsewhere, often heat-damaged in shipping.

**Stacked with:** Propranolol (Autistic users pair it with propranolol for social anxiety, one handling the physical symptoms and oxytocin the sense of connection.); Selank and Semax (Some add a small oxytocin dose to daily Selank/Semax on days with a lot of socializing.); Ketamine therapy (Taken before ketamine sessions to feel more open; anecdotal.)

## Risk profile

- **Seen harm 1/3:** Across 38 randomised trials (n=1,529) side effects at 18-40 IU did not differ from placebo; the 24-week 290-child trial had one related SAE (sedation causing a car accident at 48 IU/day), and one old case report describes water intoxication with seizures after excessive spray use plus IV fluids. Isolated and confounded.
- **Mechanism 1/3:** At obstetric IV doses oxytocin contracts the uterus and retains water, but intranasal doses are far smaller; the realistic pathways are uterine contractions in pregnancy (a contraindication) and water retention at very high doses.
- **Data gap 1/3:** An approved drug (IV) with dozens of intranasal RCTs, including a 24-week trial, though long-term daily intranasal use in healthy adults is not studied.
- **Silent harm 1/3:** Effects and side effects are felt within minutes to hours of a dose.
- **Product 1/3:** In the US it is a compounded prescription nasal spray; elsewhere grey-market sprays and vials, sometimes heat-damaged, and a few inject it.
- **Never if:** Pregnancy (uterotonic); Hyponatraemia or conditions with water retention
