- Category: Metabolic & weight (Research peptide)
- Also known as: mitochondrial ORF of the 12S rRNA type-c; 'exercise in a vial'
- Routes: Injection
- Sequence: MRWQEMGYIFYPRKLR (unmodified (16 aa, mitochondrial 12S rRNA-encoded))
- Safety tier: Strict: Avoid · Balanced: Avoid · Evidence only: Avoid
- Risk axes (0 none to 3 severe): Seen harm 2, Mechanism 1, Data gap 3, Silent harm 1, Product 3
- Positive: 41% of 53 graded reports glowing or positive, shrunk toward the 49% all-report average by 8 reports' weight
- Rank: #53 of 61 research peptides
- Graded reports: 53 (Glowing 11, Positive 10, Mixed 5, Nothing 9, Negative 8, Bad reaction 10)
- Evidence: Animal only — One small phase 1b of an analogue
- Access: Grey market
- Onset: Many feel it at the first dose, 10-70 minutes later during training (lower heart rate, easier cardio); others notice nothing until about 3 weeks in, and only under physical load.
- Lasts: One morning dose covers the day's training; a first or large dose (5 mg) can keep people awake at night, and some crash in the afternoon.
- Cost per month: ≈ €22–88 (at 210–860 mcg/day, Semax Polska at €3.41/mg)
- Dose users report: 0.83–2.1 mg/day (median 1.5 mg, n=12), ≈ €85–220 a month at that dose
- Updated: 2026-10-30

**Why Avoid under the strict bar:** Borderline: recurring reports of allergic sensitisation and anaphylaxis-like events, plus no human data on the native peptide at the doses used.

## What users report

Many 'insane' posts: doubled cardio endurance, older users lifting like 20 years ago, strongest in over-40s and unfit people. Roughly 30% feel nothing ('placebo king'). A recurring minority get exhaustion or midday crashes, and high starting doses (5 mg) cause days of soreness or fatigue. Allergy is the dominant harm theme: injection-site welts that spread over weeks, then sudden whole-body hives or lip and throat swelling, sometimes after months on the same vial. At least four Reddit users describe emergency treatment for anaphylaxis (epinephrine, IV antihistamines, ambulance), one of them after a first 1 mg dose.

**Effects:** better cardio endurance; lower exercise heart rate; energy (esp. over 40); insulin sensitivity / fat loss (with GLP-1s)

**Downsides:** fatigue/crash in some; injection-site histamine reaction; headache at high doses

**Risks:** Repeated anecdotal reports of progressive allergic sensitisation, including anaphylaxis-like reactions; No human data on the native peptide (only analogue CB4211: 20 subjects, 4 weeks); Generic: grey-market purity

**Typical use:** 1-2 mg 3x/week or 0.5-1 mg before workouts; avoid 5-10 mg starting doses.

**Getting it:** Research-peptide vials (5-40 mg); since HHS enforcement discretion (29 Oct 2026) US 503A pharmacies may compound it. The CB4211 analogue never went past phase 1.

**Stacked with:** SS-31 (Usually run first for 2-8 weeks as a mitochondrial 'repair' phase, then MOTS-c, or both at once; over-40 users credit the order for better results.); NAD+ (The near-default companion (often 25-50 mg subq); together with SS-31 the three-part 'mito stack'.); Retatrutide or tirzepatide (Added to counter GLP-1 fatigue, which confounds most fat-loss claims; one user on 8 mg retatrutide had resting heart rate climb to 100-110 after adding it.); 5-Amino-1MQ (Paired in the same cycle for energy and fat loss; users seldom can say which compound did what.)

## Risk profile

- **Seen harm 2/3:** No study has dosed native MOTS-c, but at least four independent first-hand reports describe anaphylaxis needing emergency care or epinephrine, several after months of uneventful use, which is a consistent sensitisation pattern rather than isolated anecdotes.
- **Mechanism 1/3:** A mitochondria-derived peptide acting through AMPK and nuclear gene regulation; no pharmacological pathway to serious harm is known, and the anaphylaxis points to immunogenicity of the peptide or its impurities rather than its action.
- **Data gap 3/3:** FDA found no human exposure data for MOTS-c and did not count the analogue CB4211, whose 85-subject phase 1 saw no serious events.
- **Silent harm 1/3:** Anaphylaxis is sudden and obvious, but it can arrive after months of tolerated doses, so early tolerance is no reassurance.
- **Product 3/3:** Grey-market injectable vials; US compounding began only with the October 2026 HHS enforcement discretion.
- **Never if:** Any previous reaction to MOTS-c, including hives or lip tingling; History of anaphylaxis or mast-cell disease; Pregnancy or breastfeeding (no human reproductive data)
