**Why Avoid under the strict bar:** Cardiovascular effects (BP spikes; contraindicated in CVD) and melanocortin pigmentation effects.

## What users report

Polarised. Some users (including post-prostatectomy men) report 'like a teenager' effects and intense orgasms. Others get help only 2-3 times out of 10, and one woman found it purely physical with no desire. Nausea is the main complaint, sometimes severe, plus facial flushing. About 30-40% report weak or no effect.

**Effects:** increased desire/arousal; stronger erections independent of PDE5; more intense orgasm

**Downsides:** nausea (~40% in trials); facial flushing, headache; 'physical-only' arousal; delayed onset (hours)

**Risks:** Transient blood-pressure rise and heart-rate drop after each dose; contraindicated in uncontrolled hypertension / cardiovascular disease; Focal hyperpigmentation with repeated use; Priapism risk (melanocortin class)

**Typical use:** Community starts 250-500 mcg subQ, up to the 1.75 mg label dose, max once per 24 h.

**Getting it:** Vyleesi (1.75 mg autoinjector) is a US prescription drug for premenopausal women with HSDD, capped at 8 doses a month. Men and most Reddit users buy grey-market 10 mg vials or nasal sprays (users find the sprays weak); some US clinics compound it.

**Stacked with:** Tadalafil or sildenafil (The most common pairing, PDE5 for the erection and PT-141 for desire; the ER priapism reports come from this combination.); Ondansetron or an antihistamine (Taken 1-2 hours before the dose; users say it makes the nausea and flushing manageable.); Oxytocin (Suggested and tried by women who found PT-141 purely physical with no emotional desire.); Melanotan II (Same receptor family; long-term MT2 users say MT2 gives a similar libido effect and also tans, while PT-141 users report skin darkening too.)

## Risk profile

- **Seen harm 2/3:** The Vyleesi label warns of a transient blood-pressure rise and focal hyperpigmentation (1% at up to 8 doses a month, but 38% after 8 daily doses, not always reversible); serious adverse reactions were 1.1% vs 0.5% on placebo, including one case of acute hepatitis.
- **Mechanism 2/3:** A melanocortin agonist that raises systolic pressure by up to 6 mmHg for hours after each dose and stimulates melanocytes; contraindicated in uncontrolled hypertension and cardiovascular disease.
- **Data gap 0/3:** FDA-approved since 2019 on two 24-week trials in 1,247 premenopausal women plus a 684-woman 52-week extension. The caveat is fit, not size: post-marketing use is small, and men and the daily dosing common with grey-market vials are outside the trials.
- **Silent harm 2/3:** The blood-pressure rise is silent without a cuff, and pigment changes build up over weeks of frequent dosing; nausea is immediate.
- **Product 3/3:** Most users buy grey-market vials or nasal sprays, which are far cheaper than Vyleesi; PT-141 was among the compounds in the 6,441-sample quality study.
- **Never if:** Uncontrolled hypertension or known cardiovascular disease (label); Dark skin or a history of pigment disorders, with frequent dosing; Pregnancy or breastfeeding (no human reproductive data)

## Studies

- [Kingsberg et al. 2019: bremelanotide for hypoactive sexual desire disorder, two randomised phase 3 trials (RECONNECT)](https://pubmed.ncbi.nlm.nih.gov/31599840/) (rct, n=1267, 2019): 1.75 mg as needed over 24 weeks modestly raised desire (FSFI-desire +0.35 vs placebo) and lowered distress in premenopausal women; nausea, flushing and headache in 10% or more.
- [Simon et al. 2019: long-term safety and efficacy of bremelanotide (RECONNECT open-label extension)](https://pubmed.ncbi.nlm.nih.gov/31599847/) (open-label, n=684, 2019): Over 52 weeks no new safety signals; nausea 40%, flushing 21%, headache 12%, and only 272 of 684 completed the extension.
- [Clayton et al. 2016: bremelanotide for female sexual dysfunctions in premenopausal women, phase 2b dose-finding trial](https://pubmed.ncbi.nlm.nih.gov/27181790/) (rct, n=327, 2016): 1.25-1.75 mg raised satisfying sexual events by 0.7 vs 0.2 a month on placebo and improved FSFI and distress scores.
- [Safarinejad & Hosseini 2008: salvage of sildenafil failures with bremelanotide](https://pubmed.ncbi.nlm.nih.gov/18206919/) (rct, n=342, 2008): Intranasal 10 mg helped 33.5% of men who had not responded to sildenafil vs 8.5% on placebo, with more adverse effects; the journal issued an expression of concern about this paper in 2023.
