- Category: Hormones & libido (Research peptide)
- Also known as: GRF(1-29); Geref (former US brand)
- Routes: Sublingual / Injection
- Sequence: YADAIFTNSYRKVLGQLSARKLLQDIMSR (GHRH(1-29), C-amide)
- Safety tier: Strict: Avoid · Balanced: Avoid · Evidence only: Avoid
- Risk axes (0 none to 3 severe): Seen harm 2, Mechanism 2, Data gap 1, Silent harm 3, Product 1
- Positive: 46% of 29 graded reports glowing or positive, shrunk toward the 49% all-report average by 8 reports' weight
- Rank: #41 of 61 research peptides
- Graded reports: 29 (Glowing 4, Positive 9, Mixed 4, Nothing 4, Negative 2, Bad reaction 6)
- Evidence: Approved drug — previously approved in US (Geref, pediatric GH deficiency; discontinued 2008)
- Access: Prescription (compounded)
- Onset: Sleep changes are often reported from the first nights. Recovery and body-composition effects take 4-12 weeks, and IGF-1 rises within about 2 weeks of nightly dosing.
- Lasts: Very short-acting (half-life about 10-20 minutes): each injection triggers a GH pulse of about 2 hours, so it is dosed nightly, fasted.
- Cost per month: ≈ €36–240 (researchers' estimate)

**Why Avoid under the strict bar:** GH/IGF-1 axis elevation, though the mildest of the class.

## What users report

A loyal minority rate it the budget GH option: they 'slept like a log', report better recovery, smoother skin and more energy, and see IGF-1 rise to the upper-normal range on bloodwork. Many call it weaker than CJC/Ipa or tesamorelin, and roughly a quarter notice nothing, often while starting TRT or a GLP-1 at the same time, which gets the credit. Vivid dreams, night sweats, water retention and acne recur. The most serious pattern is sensitisation: a dozen-plus first-hand accounts of hives, face and lip swelling, vomiting and collapse, sometimes needing the ER. These usually come on the second course or after earlier CJC/ipamorelin or tesamorelin use, but occasionally on the first dose.

**Effects:** sleep quality; recovery; IGF-1 into upper-normal range

**Downsides:** weaker than alternatives; flushing; injection-site reactions

**Risks:** GH/IGF-1 axis elevation (glucose, theoretical neoplasm promotion); Hypersensitivity reported across GH secretagogues

**Typical use:** 200-500 mcg nightly subQ, 5-7 nights/week.

**Getting it:** Geref was withdrawn in 2008 for commercial reasons; US telehealth and TRT clinics now prescribe compounded injections, troches and nasal sprays. Also sold by research-peptide vendors.

**Stacked with:** TRT (The most common context; clinics sell it as an add-on to testosterone, and several users credit the testosterone rather than the sermorelin for their results.); Tirzepatide or retatrutide (Taken to hold on to muscle during GLP-1 weight loss; most visible fat-loss results in these threads come from the GLP-1.); Ipamorelin (A cheaper alternative to CJC-1295/ipamorelin; one user's improved sleep turned into 3 am waking and a pounding heartbeat once ipamorelin was added.)

## Risk profile

- **Seen harm 2/3:** The Geref label lists only flushing and injection pain and FDA confirmed its 2008 withdrawal was commercial, but at least four independent first-hand reports describe anaphylaxis-type reactions (lip and face swelling, collapse with low blood pressure, an emergency visit), several in people sensitised by other GHRH or ghrelin-mimetic peptides: a consistent pattern.
- **Mechanism 2/3:** A GHRH fragment that raises GH and IGF-1, though more weakly than the other secretagogues and with feedback preserved; the credible concerns are the IGF-1 axis's glucose and tumour-promotion effects.
- **Data gap 1/3:** Approved in the US from 1990 to 2008 for diagnosis and paediatric GH deficiency, on small paediatric and diagnostic studies; no long-term adult data.
- **Silent harm 3/3:** Tumour promotion from raised IGF-1 would be silent and delayed, though IGF-1 and glucose can be tracked on blood tests.
- **Product 1/3:** Mostly prescribed through US telehealth and compounding pharmacies; grey-market vials are the alternative.
- **Never if:** Active cancer or a history of cancer; A previous reaction to CJC-1295, ipamorelin, tesamorelin or another GH secretagogue; Diabetes or impaired glucose tolerance; Pregnancy or breastfeeding (no human reproductive data)

## Top reports

- Glowing, recovery: “I started taking sermorelin about three weeks ago as a way to recover a little faster and sleep better because, let's face it, I'm not 22 anymore. Personally, I've noticed a massive difference in sleep, mood, energy levels, and performance in the gym.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1sca4w0/sermorelin_so_far/)
- Glowing, energy: “for the last couple of years (maybe 2) I’ve just felt like I’m dragging. I would sleep all of the time and it was never enough. The gym basically became nonexistent and I was a 5x/week gym goer. Ever since starting this, I just feel like my old (younger) self.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1sl6w1n/sermorelin_experience/)
- Glowing, recovery: “I’m 35 (F) naturally strong but went to superhuman with Sermorilin and could always do like 70-90 lbs on adductor/abductor and they went to 145 lbs after 20 years of solid lifting with little more gains. Superhuman strength and sleep.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1sca4w0/sermorelin_so_far/ogzth41/)
- Glowing, energy: “I also have been mostly bed bound for the past 3+ years from CFS & POTS. I have major post exertional malaise. I’ve been taking the sermorelin for almost a month now. & I’m getting out of bed again. It has been a small miracle l.” — [r/LadiesHRTwithGLP1](https://www.reddit.com/r/LadiesHRTwithGLP1/comments/1pf0p47/anyone_use_sermorelin/ogcoe4o/)
- Positive, sleep: “my experience on Sermorelin was pretty good. I slept like a log, and that benefit alone made it worth it. Plus it’s cheap.” — [r/BodyHackGuide](https://www.reddit.com/r/BodyHackGuide/comments/1sca4w0/sermorelin_so_far/oe9vu6j/)

## Studies

- [Thorner et al. 1996: once-daily subcutaneous GHRH(1-29) in GH-deficient children](https://pubmed.ncbi.nlm.nih.gov/8772599/) (open-label, n=110, 1996): 30 mcg/kg at bedtime roughly doubled height velocity (4.1 to 8.0 cm/yr at 6 months) with no rise in fasting glucose or excessive IGF-1; the basis for the Geref approval.
- [Corpas et al. 1992: GHRH(1-29) twice daily reverses decreased GH and IGF-I levels in old men](https://pubmed.ncbi.nlm.nih.gov/1379256/) (controlled, n=19, 1992): Two weeks of 1 mg twice daily brought GH pulses and IGF-1 in 10 older men up to the levels of 9 young controls, without changing fasting glucose.
- [Vittone et al. 1997: single nightly injections of GHRH(1-29) in healthy elderly men](https://pubmed.ncbi.nlm.nih.gov/9005976/) (open-label, n=11, 1997): Nightly injections raised nocturnal GH release but not IGF-1, and did not change weight, DEXA muscle or fat; some strength measures improved.
- [Khorram et al. 1997: long-term [Nle27]GHRH(1-29) in age-advanced men and women](https://pubmed.ncbi.nlm.nih.gov/9141536/) (controlled, n=19, 1997): 16 weeks of a close sermorelin analogue nightly raised IGF-1 and skin thickness in both sexes and lean mass, insulin sensitivity, well-being and libido in men only; sleep quality did not change.
- [Walker 2006: Sermorelin, a better approach to management of adult-onset growth hormone insufficiency?](https://pubmed.ncbi.nlm.nih.gov/18046908/) (review, 2006): Argues GHRH therapy preserves the pituitary's own feedback and pulsatility, making overdose less likely than with GH; evidence in adults is small and short.
