Users report
The most-discussed product by far is BIOEFFECT's barley-grown EGF serum and eye serum, then Le Mieux, Klairs and assorted Korean EGF essences. About 60% describe a gain: glowier, plumper, silkier skin within days to weeks, firmer upper eyelids and crepey under-eyes over months (several eye-serum users of 2-3 years), faster calming of redness after surgery or irritation, and in one case longer lashes. About a fifth used a whole bottle, sometimes three months of BIOEFFECT, and saw nothing worth the price ($150-180 per 15 ml), and others found the face serum greasy or broke out from the rich Korean creams. Nearly everyone also uses tretinoin, acids or other actives, so the EGF's own share is unclear, and several threads raise the cancer question; people with skin-cancer history or suspicious moles say they avoid it. Exosome and conditioned-media serums (TNS, Plated, Neogenesis) are not counted here. Felt nothing: ~21% of reports (5/24).
Best effects
Glowier, plumper, more hydrated skin, sometimes overnight · firmer, less crepey eyelids and under-eyes over months (eye serums) · smoother texture; some say shallow acne scars softened · faster calming of redness after procedures or irritation
Downsides
Expensive for an effect many can't see ($150-180 per 15 ml for the best-known serum) · about a fifth notice nothing after a full bottle or months · greasy feel; breakouts from some rich EGF creams and essences · fragile protein: brands say to avoid acids, vitamin C and oils at the same time
Watch
Theoretical tumour promotion: EGF does not start cancers, but in animal co-carcinogenesis studies it can promote tumours, so applying it to a skin cancer, precancer (actinic keratosis) or suspicious mole could in principle speed its growth; this would be silent and delayed. On intact skin very little of this 6 kDa protein gets past the barrier, but microneedling, peels, lasers and broken skin deliver it straight to living cells.
Evidence
Small human studies — the molecule is an approved wound-healing drug in Korea (topical rhEGF for diabetic foot ulcers, phase III placebo-controlled trial) and elsewhere, but the cosmetic evidence is a 29-woman open-label study of a barley-EGF serum and a 20-woman pilot RCT comparing two growth-factor serums, both without placebo; how much reaches living skin through an intact barrier is doubtful
Caveat
This verdict covers EGF serums on intact skin, where a 6 kDa hydrophilic protein barely penetrates and there is little sign it reaches the dividing cells it acts on; that low exposure is why it is not excluded. Used on microneedled, peeled, lasered or broken skin it does reach living epidermis and dermis, and a direct growth-factor agonist at that exposure would fall under the strict bar's exclusion (mechanism 2 with silent, delayed harm). Skip it entirely on or near skin cancers, actinic keratoses or changing moles, and after a skin-cancer diagnosis. Efficacy evidence is thin and mostly industry-linked; 'sh-oligopeptide-1' on a label says nothing about how much active protein survived formulation.
Typical use
Finished serums at a few ppm of EGF (one Korean ampoule advertises 10 ppm), 2-4 drops once or twice daily on clean, dry, intact skin, waiting before acids or oils.
Getting it
Over-the-counter cosmetic: BIOEFFECT EGF Serum and eye serum (barley-produced), Le Mieux EGF, Klairs Midnight Blue Youth Activating Drop, Isntree, Easydew and many Korean EGF essences and creams; EGF drug products (Easyef in Korea) are prescription wound treatments.
Stacked with
Tretinoin: The usual base; users alternate nights and say EGF calms retinoid irritation. · microneedling: Commonly applied right after needling or peels; this pushes a growth factor past the barrier and is outside this entry's verdict.

Seen harmMechanismData gapSilent harmProduct

Seen harm 0/3
No serious harm documented despite real exposure: topical rhEGF has been used on open wounds since 1989 with 6-24 month follow-ups and a 15-year Cuban burn-patient survey showing no excess cancer, a 1,172-patient radiation-dermatitis series reported no severe adverse events, and EGF cosmetics have been mass-market for two decades. First-hand reports are breakouts and greasiness only. Berlanga-Acosta et al., EGF in clinical practice: safety implications (Int Wound J 2009) ↗The use of epidermal growth factor in dermatological practice (Int Wound J 2023) ↗
Mechanism 1/3
EGF is itself a growth factor and a direct EGF-receptor agonist, and in animal co-carcinogenesis work it promotes (though does not initiate) tumours, so on open skin this would be a 2. But the dominant use is a serum on intact skin, where a 6 kDa hydrophilic protein barely crosses the stratum corneum, so under the topical convention it scores 1; applied after microneedling, peels, lasers or on broken skin it reaches living cells and would score 2, which with latency 3 is excluded under the strict bar. Berlanga-Acosta et al., Int Wound J 2009 (tumour promotion without initiation) ↗Challenges and effective routes for formulating and delivery of EGF in skin care (Int J Cosmet Sci 2020) ↗
Data gap 1/3
The same molecule is an approved wound drug in Korea (Easyef, topical rhEGF; phase III placebo-controlled trial) and has controlled wound trials well over a hundred patients, but no stringent-regulator approval and no CIR assessment; for chronic cosmetic use on the face there are only a 29-person open-label study and a 20-person pilot. Park et al., topical EGF spray phase III RCT in diabetic foot ulcers (Diabetes Res Clin Pract 2018) ↗Schouest et al., barley-EGF serum open-label study, n=29 (J Drugs Dermatol 2012) ↗
Silent harm 3/3
The harm that matters is promotion of an existing skin cancer, precancer or atypical mole, which would be silent and delayed and invisible to testimonials; the everyday reactions (breakouts) are immediate and minor. Berlanga-Acosta et al., Int Wound J 2009 ↗
Product 0/3
Bought as finished over-the-counter serums and creams (BIOEFFECT, Le Mieux, Korean essences); DIY EGF concentrates and microneedling 'slips' are a minority channel.
Never if
active or past skin cancer (melanoma, basal or squamous cell carcinoma), actinic keratoses, or changing or atypical moles in the treated area · use on microneedled, peeled, lasered or broken skin (delivers the growth factor to living cells; outside this entry's verdict) · pregnancy and breastfeeding (no reproductive data for chronic cosmetic use)

Sequence beads

Each bead is one amino acid in the peptide's chain, coloured by its side chain: grey for non-polar, cyan for polar, blue for positively charged and fuchsia for negatively charged. Length and mix tell related peptides apart at a glance, from a two-residue dipeptide like carnosine to a 43-residue protein like thymosin beta-4. The colours describe chemistry only and predict neither effect nor safety.

Report grades

Every first-hand Reddit or X report is graded on its own, from glowing through positive, mixed, nothing and negative to bad reaction, and the bar shows how those grades split. Posts that look vendor-adjacent stay on the peptide's page but are left out of the counts.

GlowingPositiveMixedNothingNegativeBad reaction

% positive

The share of a peptide's graded Reddit and X reports that are glowing or positive, vendor-adjacent posts left out. A peptide with few reports is pulled toward the average of all reports, as if it had eight average ones besides its own, so two glowing posts cannot top the list. Kept peptides rank by the same reports' mean grade, shrunk the same way, and excluded ones follow in that order.

Researcher estimate

No first-hand report of this peptide has been graded yet, so the figure is the researchers' own rating out of 10 from what they read about it. It ranks alongside the computed scores until reports arrive.

Safety tier

Each peptide is tiered clean, caution or excluded from its five risk axes under the chosen safety bar. An excluded peptide has a plausible route to serious harm, so it stays listed with the reason but ranks behind every other entry, however good its reviews.

Safety bar

The safety bar sets how much risk it takes to exclude a peptide. Strict: out on any serious mechanism with little human data, or any documented serious harm. Balanced: out on documented serious harm (unless it is obvious, reversible and from a regulated product), or a serious mechanism whose harm would be silent with no human data at all. Evidence only: out only on documented serious harm in humans that is lasting, delayed or from grey-market product.

Seen harm

Documented serious adverse events in humans, from 0 for none despite real exposure to 3 for deaths or several well-documented serious harms. Serious means a hospital or emergency visit or harm lasting weeks, so a headache or a bad night does not count.

Mechanism

A plausible biological route to serious harm at the doses people use, such as growth-factor signalling, hormone-axis suppression, heart or blood-pressure effects or immune activation. It runs from 0 when none is known to 3 when a dangerous pathway has been shown in living animals.

Data gap

How little human safety data exists for the molecule actually sold, from 0 for an approved drug with large trials and years of use to 3 for no human safety data at all. A modified analogue does not inherit its parent's trials.

Silent harm

Whether the feared harm would show quickly or stay hidden, from 0 when it is obvious and goes on stopping to 3 when it is silent and delayed, like tumour promotion or slow organ damage. User reports cannot reveal a harm that takes years to appear, so a high score here limits what good reviews can tell you.

Product

The risk of what people actually buy, from 0 for a regulated pharmacy or supplement product taken by mouth or on the skin to 3 for a grey-market injectable. In one dataset of 6,441 grey-market samples, 42–71% failed basic quality criteria and 15% carried endotoxin.

Evidence

The strongest kind of study behind the peptide's effects, from an approved drug through human controlled trials and small human studies to animal work or nothing of its own. Animal results often fail to carry over to people.

Access

How people get it: as an approved drug, a compounded prescription, an over-the-counter supplement or from the grey market, where nobody checks the vial. The channel decides both the legal position and the risk of a bad batch.

Price per month

Thirty days of the typical dose, from its low end to its high end, priced at the listing a careful buyer would choose: in stock, from a shop scoring 4 of 5 on trust (third-party tested where no score is set), failing that 3, a pure pack before a blend, a European shop first, then the cheapest per mg. The range is the dose's, not the shops'. A per-dose amount counts once a day unless the dose says otherwise. Prices are in euros at ECB rates of 2026-10-29, before shipping; the shop table keeps each listing's price per mg, per gram for bulk supplements. Where no listing can be priced the researchers' own estimate stands in, and says so.

Dose users report

The middle half of the daily doses stated in at least five first-hand reports, vendor posts left out. Where its middle sits more than twice above or below the declared dose, the month is priced at it as well, since what people take is what they pay for.

Testing

Whether the shop publishes lab results: third-party tested means an independent lab checks each batch, own COA means the shop's own certificate of analysis, and no public tests means neither. A certificate shows purity, not safety.

Added peptide

Some packs sold under one peptide's name hold a second one, such as 5 mg GHK-Cu in a nasal kit, stated only in the description. Sprayed in the nose or injected it reaches the blood like an injection, so it carries the risk of that route and is tiered as the injectable form, whatever its tier on the skin. A pack whose page gives no contents list may hold more than its name.
